September’s Most Inspiring Moments

Every day, our nurses pick up the phone, not knowing what the next call will bring. It might be a worried patient, a family member with urgent questions, or someone who simply needs a kind voice on the other end of the line.

These aren’t box-checking tasks, they are moments where care becomes deeply personal. Moments where a nurse’s words help someone feel safe, supported, and seen. Our nurses don’t just monitor data, they show up for patients with empathy and listen when it matters most.

September was full of these moments. Patients found hope after hard news, regained their independence, celebrated major health milestones, and finally felt heard.

At Vivo Care, this is what “care between visits” truly means. These are real conversations, happening in real time, made possible by the dedication and compassion of our nationwide nursing team.

Regaining Independence – Johanna Osorio

One of Johanna’s patients went from frequent hospital visits and daily falls to driving, gardening, cooking, and keeping his home tidy again. His sister shared that this program not only restored his health but brought their family closer together.

“He is driving, gardening, cooking his meals, and keeping his home tidy like before. … She thanked me for helping so much and said he wouldn’t be here without our RPM & CCM program.” – Johanna Osorio

A Listening Ear in a Hard Time – Pearl Chung

Pearl spoke with a patient who had recently lost her son. She shared that no one had checked in on her — except Pearl.

“I took extra time to listen to her about her grieving process, gave her consolement, and we even shared some laughs. … It’s heartwarming to be reminded that often our reaching out is beyond just a health check point — it can mean so much more.” – Pearl Chung

Clearing the Way for Care – Tricia Milligan

Tricia’s patient was overwhelmed by insurance denials and clinic billing issues that were delaying her treatment.

“We talked through her options and decided the best thing to do was to follow up with her doctor. … She told me that the billing issues have been resolved and she received her first iron infusion on Wednesday. She told me that I was a God send.” – Tricia Milligan

Comfort in a Moment of Crisis – Bianca Patrick

During a routine call, Bianca’s patient became emotional and started to cry while sharing her struggles with a new medication regimen and difficulty keeping her blood pressure under control. Bianca stayed calm and reassuring, offering a steady presence as the patient opened up.

After the call, Bianca sent educational materials on stress awareness and management to reinforce what they discussed.

“While I felt it deeply myself, I focused on remaining supportive, calm, and reassuring for her. … She gradually calmed down and expressed sincere appreciation for the call and my concern.” – Bianca Patrick

Celebrating a Cancer-Free Milestone – Michelle Graves

Michelle has been calling this patient for two years, supporting him through his health journey.

“He told me first thing, he is CANCER FREE!!! … He’s gone from smoking 2 packs of cigarettes a day to an average of 5-8 a day. This has absolutely made my day.” – Michelle Graves

Closing Reflection

Across all 50 states, our licensed nurses show up every day as an extension of our clinic partners, answering questions, easing worries, and celebrating milestones right alongside the patients we serve.

These stories are reminders of what’s possible when remote care is done right. Our nurses are redefining what it means to feel seen, heard, and cared for between visits.

These September moments are just a glimpse of the thousands of calls, check-ins, and quiet victories that happen every month. And while not every clinic uses our clinical monitoring team, the lessons we learn from these conversations shape everything we do — from how we design our software to how we support teams running their own programs.
