Bringing Remote Care to Life
Octobers Most Inspiring Moments
At Vivo Care, every call matters. Our nurses step into the moments between visits. They listen. They act. They keep patients connected as they work on behalf of our provider partners to deliver compliant, engaging Remote Patient Monitoring and Chronic Care Management programs.
Our US-based team of over 100 nurses does more than monitor data and follow care plans. They open doors. They build trust. They transform care with every interaction.
Here are a few moments from October that show what remote care feels like when it’s working as it should. To see more heartwarming stories like this, please check out last month’s edition of Vivo Care Moments that Matter.
Patient stories are the heartbeat of our work – they remind us why every call, every reading, and every care plan matters. At Vivo Care, these moments of connection are a testament to the skill and compassion of our nursing team, and they illuminate the profound impact RPM and CCM services can have on people’s lives
Kat Baker – Director Clinical Operations at Vivo Care
“Thank You for Not Giving Up” — Eva Daizi
When a woman in her mid-40s sent in a blood pressure reading of 176/90 and later 180/100, nurse Eva called right away. The patient was symptomatic and under stress. Eva didn’t hang up until she knew the patient would see her provider that same day.
The patient later said, “Thank you for not letting me dismiss these symptoms and pushing me to get medical attention.”.
“Finding Hope in Hunger” — Rebecca Sizemore
Rebecca discovered a patient surviving on one meal a day and paying neighbors for rides to medical visits. She connected her with local food banks and transportation support.
The patient texted afterward: “Thank you so very much for helping me with everything. I appreciate you so much.”
Sometimes the most meaningful care begins with meeting the basics.
“A Laugh That Lightened the Call” — Hannah Robbins
When Hannah called about a critical alert and asked about symptoms, the patient replied: “I have none of those symptoms — besides being married, maybe that’s it.”
A quick laugh can ease tension, build trust, and make care feel human again.
“You’re My Favorite Nurse” — Kaila Zamble
During a routine wellness check, Kaila was greeted with, “Good morning! How’s my favorite nurse doing?”
Without ever meeting face-to-face, these patients still feel seen — proof that consistent, caring voices matter.
“Calming Therapy from the Porch” — Vonda Soileau
Vonda’s patient told her, “Every time I talk to you it’s so relaxing, like we’re sitting on the front porch looking out over your pond.”
When a nurse’s voice becomes a source of calm, it shows how deeply connection fuels healing.
“Helping a Patient Rebuild Confidence After Loss” — Kelsey Robinette
Kelsey’s patient had lost her husband, was closing the family business, and recovering from a stroke. She struggled with balance, fear of falling, and loneliness.
Kelsey guided her toward physical therapy, vision care, and small, practical help at home — even finding someone to manage her yardwork.
Real care isn’t always high-tech. Sometimes it’s simply helping someone stand tall again.
“Grace in the Face of Hardship” — Hinal Patel
Hinal spoke with a woman caring for her husband with dementia and Parkinson’s, while facing her own illness and a daughter’s advanced cancer.
Despite it all, she radiated hope — making a vision board, leaning on faith, and finding strength each day.
Her story was a reminder that emotional resilience is its own kind of medicine.
“A Community of Care — and Kindness” — Maria Saenz
Maria shared a story about a patient who still remembered nurse Tara months later, asking if they could send her a thank-you gift.
It was a small gesture that captured a big truth: when you lead with compassion, patients never forget your care.
“Joy Still Knocks at the Door” — Susan Ngute
One of Susan’s patients, who lives in a senior community, told her she had her candy ready and was dressing up for Halloween — even if no kids came by.
She said, “Sometimes it feels like we’re forgotten.”
It’s a reminder that joy matters at every age — and connection can start with a simple knock on the door.
Closing Reflection
Across every conversation, our nurses bring connection, compassion, and calm to the moments that matter most. These stories are only a glimpse of the thousands of meaningful calls happening each month across Vivo Care.
Whether it’s a critical reading, a shared laugh, or a quiet moment of encouragement, each story reflects why we do what we do — to make care between visits meaningful, measurable, and human.
Thank you for helping remote care come alive.
Whether a partner uses Vivo Care for software, clinical services, or both, our mission is the same: to make care between visits meaningful and measurable. We couldn’t be prouder of our nationwide nursing team for showing what’s possible when remote care truly comes alive.
