  • Composite cardiovascular health scores (covering BP, cholesterol, A1c, BMI, and smoking status) improved from 4.5 to 5.2 , with two-thirds of that gain driven by better blood pressure control.
  • Patients with high cholesterol saw notable reductions in total cholesterol, LDL, and triglycerides.

While the study was observational and had incomplete data for some participants, the results point to the strong potential of combining RPM with coordinated team support.
