Remote patient monitoring uses connected, FDA-cleared cellular devices to bring vital signs from the home to your care team, with no home internet required.

The software

The Vivo Care platform gives your team one place to see incoming readings, document the care, and generate the billing. Reporting and documentation are built in, so the monthly management time and the required interactive contact are captured for a clean claim. Choose self-managed, where your team runs the platform, or fully managed, where Vivo Care care navigators handle the monitoring.

The devices

FDA-cleared, cellular-connected devices ship directly to patients and pair automatically over the cellular network. No WiFi, smartphone, or setup is required, which is what makes the program work in rural and frontier homes. The catalog covers the vitals that drive chronic disease: blood pressure monitors, cellular glucose meters, weight scales, and pulse oximeters. For patients who need it, a cellular gateway hub is available that connects additional devices over the same cellular network, so a home with no internet is never a barrier to enrollment.

Which patients benefit

Patients your provider determines could avoid an admission with closer monitoring. Common conditions include diabetes, COPD, heart failure, asthma, pneumonia, and high-risk pregnancy. Your provider directs the care plan.

What your practice can bill

Remote patient monitoring is reimbursable under both Medicare and Colorado Medicaid. Your practice bills for the care it provides and keeps the reimbursement, while Northeast Health Partners covers the program cost.

Per enrolled patient, that is roughly $108 to $150 per month under Medicare and $57 to $88 under Colorado Medicaid. Device supply bills 99454 for 16 or more days or 99445 for 2 to 15 days, not both. Management bills 99457 for the first 20 minutes, plus 99458 for added documented time, or 99470 when a month reaches 10 to 19 minutes of documented management time with at least one real-time patient or caregiver contact. Under Colorado Medicaid, the two new 2026 flexibility codes (99445 and 99470) are well-valued by Medicare but nominal under Medicaid, so practices bill 99454 and 99457. CY2026 Colorado fee-schedule amounts. Payment depends on documented time and medical necessity.
