Vivo Care
The software
The Vivo Care platform gives your team one place to see incoming readings, document the care, and generate the billing. Reporting and documentation are built in, so the monthly management time and the required interactive contact are captured for a clean claim. Choose self-managed, where your team runs the platform, or fully managed, where Vivo Care care navigators handle the monitoring.
The devices
FDA-cleared, cellular-connected devices ship directly to patients and pair automatically over the cellular network. No WiFi, smartphone, or setup is required, which is what makes the program work in rural and frontier homes. The catalog covers the vitals that drive chronic disease: blood pressure monitors, cellular glucose meters, weight scales, and pulse oximeters. For patients who need it, a cellular gateway hub is available that connects additional devices over the same cellular network, so a home with no internet is never a barrier to enrollment.
Which patients benefit
Patients your provider determines could avoid an admission with closer monitoring. Common conditions include diabetes, COPD, heart failure, asthma, pneumonia, and high-risk pregnancy. Your provider directs the care plan.
What your practice can bill
