Every visit. Every day between. One connected journey.
Better care between visits. Less work behind every visit.
Remote care that starts with the provider. Together, Vivo Care and Onpoint Healthcare Partners extend what providers and their teams can do, inside the office and between visits, without adding to the team’s workload.
Starts with the provider. Continues at home. Lands where the provider works.
Your patients qualify for remote care. Why aren’t they enrolled? What happens to them between visits? Who handles the work remote care creates in your office? Here is what one connected journey looks like for Robert Kelso, a patient with high blood pressure.
In the visit
His order is ready in the chart before he arrives.
His provider explains how the program fits his treatment plan. Robert consents, face to face, and the order goes in electronically, in one step. When enrollment starts with the provider, 40% more patients say yes.
Every day between
Vivo Care and Onpoint take it from there.
Clinical staff support Robert between visits, his provider’s team or our optional U.S.-based nursing team. His blood pressure climbs, and the prior authorization is handled so new meds start on time. His labs come back, reviewed and back in his chart.
Where the provider works
What happens at home comes back to his provider’s team, handled.
Inbox triaged. Prior authorizations handled. Lab results reviewed. Visit notes drafted. With medical experts in the loop, and clinical decisions staying with the treating care team.

“Practices don’t need one vendor for the visit and another for the time in between. They need the care to connect. We are building that the responsible way, with licensed experts in the loop.”
Ryan ClarkPresident and CEO, Vivo Care“When we looked at what came next for Onpoint, the question was who could take our customers further. Vivo Care already owns the days between visits, so together our customers get one team from the exam room to the patient’s home.”
Sean CarrollChairman of the combined organization“We built Onpoint to take routine work off providers and their teams so they can practice medicine. Combining with Vivo Care carries that same discipline into the patient’s home.”

-
What changes for current customers?
Nothing changes today. Contracts, services, and points of contact stay in place.
-
What is point-of-care enrollment?
In integrated practices, the chart prompts the provider to raise remote care during the visit, and the patient enrolls in the room. From there, a licensed care team follows the patient at home and documents back to the chart.
-
Which EHRs do you work with?
Remote care works on any EHR, and providers can refer patients directly. Point-of-care enrollment is live on athenahealth, with eClinicalWorks to follow in November.
-
How do you use AI?
AI is how we deliver efficiently, with medical experts in the loop. Every workflow is proven before more automation is added, and clinical decisions stay with the treating care team.
-
Can we use our own nurses?
Yes. Practices can use Vivo Care’s licensed care team or their own nurses, and a software-only option is available.
